To register by mail or fax, open the Registration Form (.pdf) by clicking on the link below. Print out the registration form and fill out the requested information. When the form is complete, fax the form to 508-349-7908 or mail it to the address below.

To register online, fill out the membership and program information below and click the “Continue to PayPal” button. You will be taken to the PayPal site for the secure processing of your payment. The information about the GISC program for which you are registering is automatically communicated to the PayPal site.

PayPal is a secure site used for the processing of online payments. PayPal will ask you for the necessary financial information (credit card of banking information) to process your payment to GISC. A record of this payment will be sent to your email address. It will show that the payment has been made to GISC.
