GISC REGISTRATION FORM

Before registering for the Cape Cod Training Program you need to complete an application, accompanied by a $100 non-refundable application fee and be accepted to the program. Please select and open the CCTP Application Form by clicking on the appropriate link below. Print and complete the application and fax it to 508-349-7908 or mail it to the address below. Once you are notified that you have been accepted, you may then register and pay online using the PayPal process below. For all other programs, you may register immediately by mail, phone, fax, or online.

To register online, fill out the membership and program information below and click the “Continue to PayPal” button. You will be taken to the PayPal site for the secure processing of your payment. The information about the GISC program for which you are registering is automatically communicated to the PayPal site.

PayPal is a secure site used for the processing of online payments. PayPal will ask you for the necessary financial information (credit card of banking information) to process your payment to GISC. A record of this payment will be sent to your email address. It will show that the payment has been made to GISC.
